If you are a registered parishioner of St. Andrew by the Bay and you are requesting for your child to receive a sacrament outside of the parish, please use the form below to request a Letter of Permission. Upon completion, the letter will be signed by our Pastor and forwarded to the Church where the Sacrament/Preparation will take place.​
